The Annual Strategic Grant cycle by Cape Cod Healthcare (CCHC) aims at addressing the unmet health needs of Barnstable County residents. It is an annual competitive process inviting non-profit community organizations in Barnstable County to submit proposals that align with the needs identified in the Community Health Needs Assessment (CHNA). These proposals should target programs or projects that improve physical and/or mental health, address unmet public health needs, and promote health equity, specifically focusing on the areas prioritized by the CHNA, such as Behavioral Health, Healthcare Access, Housing, Health Equity, Economic Stress & Instability, and the impacts of Covid. Successful proposals will effectively demonstrate how the project will enhance the health of Barnstable County’s population, addressing one or more of the set priorities and cross-cutting themes.
