The Child Neurology Foundation (CNF) aims to enhance the patient care experience through its Care Advancement Grants. Specifically, this grant program supports projects that improve transition of care or care coordination for patients. With a financial backing of $26,000 each, a total of thirteen (13) grants will be disbursed to institutions and/or clinics. These projects should aim for tangible improvements within a year, and may span areas such as research, outreach activities, software purchases, and labor costs.
