The mission of the Colorado Health Foundation is to improve the health of Coloradans. It operates on three cornerstones: serving Coloradans with less power, privilege, and income, especially prioritizing those of color; being informed by the community it aims to serve; and focusing all efforts towards creating health equity.
The grant offers support to community members and organizations that work in partnership to address and solve health-related challenges identified by the community itself. Particularly focusing on advancing health equity, the foundation gives priority to proposals centering on communities of color. It seeks to fund projects that enhance health equity and lead to healthier communities by empowering community members in decision-making processes. The Foundation encourages applications for a variety of programs and projects that are designed to: address a health challenge, develop and implement a solution, leverage existing strengths and resources, and improve health equity. Through this approach, the Foundation underlines the importance of community-led initiatives in creating sustainable impacts on health challenges.
