Con Alma Health Foundation aims to improve health and advance health equity for New Mexico’s diverse communities. They strive to ensure fair opportunities for healthy lives by focusing on access to healthcare, reducing health disparities, and supporting culturally appropriate services.

Type of Support

Overview

This grant program is a cooperative effort between Con Alma Health Foundation and the Hospital Auxiliary of Los Alamos Medical Center. It targets nonprofits or fiscal sponsors with a 501(c)(3) status aiming to enhance the health and wellness of residents in Los Alamos, Rio Arriba, and northern Santa Fe counties. Eligible projects should address health disparities affecting underserved groups in these regions by improving health conditions, bolstering safety-net organizations, engaging in policy advocacy, and fostering community-led health and wellness initiatives. Specifically, successful initiatives will work towards expanding health equity by eliminating disparities in health quality, access, and risks, as well as by increasing opportunities for vulnerable populations. These objectives should align with addressing social determinants of health as defined by the CDC's Healthy People 2030 or community-identified factors.

Review Criteria

In New Mexico, the focus lies on supporting communities that have faced historical marginalization or trauma due to factors such as race, language, gender identity, sexual orientation, or religious beliefs. This includes individuals who are uninsured, underinsured, and those who do not have adequate access to medical services. Additionally, there is a dedicated effort to assist people living with disabilities and those grappling with chronic health issues and the associated disparities in health outcomes. Attention is also directed towards supporting immigrant populations, communities situated in rural and border areas, and those experiencing economic hardship.
