The Healthcare Foundation of Northern Lake County is committed to improving access to quality health care for Lake County’s low-income residents. This mission is pursued through supporting programs aimed at uninsured or underinsured individuals and families and underserved neighborhoods, and by enhancing the capacity of programs, organizations, and systems serving those most in need.
The grant program focuses on broad goals such as increasing program effectiveness, ensuring organizational sustainability, and leveraging healthcare system resources. Specific causes supported include improving health status through access to comprehensive care, reducing healthcare costs, and increasing non-profit and health system capacity. The grant program outlines five main strategic areas: Clinical care, Linkage to care, Scholarships, Organizational capacity building, and Systems capacity building. Each strategy targets a unique aspect of healthcare improvement, from direct clinical services and education to enhancing organizational and systems efficiency in northern Lake County. Funding ranges typically from $25,000 to $100,000.
