The Intermountain Community Care Foundation is dedicated to supporting Intermountain’s health priorities, improving access to healthcare, and increasing healthy behaviors among low-income, uninsured, or medically underserved populations in Utah and southern Idaho. The foundation achieves its mission by identifying and supporting programs and services that enhance access to healthcare for these vulnerable populations, with the ultimate goal of helping individuals live the healthiest lives possible.
The grant program guided by the Intermountain Community Care Foundation focuses on improving community health in alignment with Intermountain Healthcare’s priorities for 2020 through 2022, as identified through a Community Health Needs Assessment in served regions. The program offers grants in two main categories: Agency Health Priority Grants and Social Determinants of Health Grants, each with specific criteria. The Agency Health Priority Grants target programs that address mental well-being, prevent avoidable disease and injury, improve air quality, enhance access to comprehensive healthcare, and increase healthy behaviors. The Social Determinants of Health Grants focus on addressing key areas like food insecurity, housing instability, interpersonal violence, transportation issues, and utility needs. The goal is to support initiatives that align with Intermountain’s health priorities to foster healthier communities.
