The mission of the Office of Minority Health is to improve the health of racial and ethnic minority populations through the development of health policies and programs that will help eliminate health disparities.
The grant program aims to support community level innovations that tackle barriers related to social determinants of health (SDOH), thereby increasing the use of preventive health services and advancing progress toward Leading Health Indicator (LHI) targets. LHIs are chosen from the prioritized objectives of Healthy People 2030 (HP2030), focusing on driving actions that improve health and well-being. This grant specifically addresses the impacts of the environments where people grow and interact, on a wide array of health, functioning, and quality-of-life outcomes and risks, as outlined in HP2030.
