The mission of the Colorado Health Foundation is to improve the health of Coloradans. It operates on three cornerstones: serving Coloradans with less power, privilege, and income, especially prioritizing those of color; being informed by the community it aims to serve; and focusing all efforts towards creating health equity.
The grant aims to build a pipeline of primary care providers that reflect Colorado's diverse communities. It supports individuals at various stages of their career, including high school graduates not pursuing college, early career individuals aiming to enter or advance in primary care, and those outside the healthcare field wanting to transition into primary care. The goal is to enhance the primary health care workforce in safety net systems and serve rural and frontier communities, aligning with the foundation's commitment to health equity. Grants of up to $600,000 over three years can be awarded, and proposed programs or projects must align with the foundation's cornerstones.
