    Overview

The FY 2024 Promoting the Integration of Primary and Behavioral Health Care: States program by SAMHSA/CMHS aims to enhance the integration of behavioral and primary physical health care to improve the wellness and physical health status of individuals. This grant program focuses on encouraging full collaboration in clinical practices between physical and behavioral health, supporting the enhancement of integrated care models, and promoting the implementation and improvement of bidirectional integrated care services. This encompasses evidence-based or informed practices for screening, assessment, diagnosis, prevention, treatment, and recovery services for mental and substance use disorders, along with co-occurring physical conditions and chronic diseases. Recipient states are required to collaborate with community programs and health centers to develop and execute an integrated care program plan, with the overarching goal of improving health outcomes for individuals with behavioral health conditions.
