Vitalyst Health Foundation aims to enhance the health of individuals and communities in Arizona by fostering partnerships, providing capacity building resources, and grant support to aligned organizations. Their mission focuses on connecting, supporting, and informing efforts to foster community health improvements across Arizona.
Systems Change Grants are designed to foster sustainable changes that are not reliant on grant funds or external expectations but are a priority within organizational or cross-system policies, culture, communication, or practices. The grants target efforts that effectively tackle complex community health challenges and diminish structural and/or systemic disparities in health outcomes or chances. Specifically, they support policy changes or practice adjustments that streamline processes, such as simplifying the steps required for enrollment in a state healthcare system, addressing unequitable practices, or creating more coordinated systems. Eligible projects must involve collaboration among two or more partners, with at least one being an Arizona-based nonprofit serving Arizonans. Projects should aim for a specific policy, systems, or environmental change, improve specific health outcomes, incorporate a health equity perspective, involve an identified population, have a budget that aligns with the work scope, and propose a sustainable solution that will persist beyond the grant period. Grant awards are offered for three-year periods, with a maximum total of 175,000 USD.
