Applications should be submitted in the form of a completed Cooper Tire & Rubber Company Matching Gift Program Form. The top half of the form is to be completed by the employee and sent with the employee's donation to the receiving organization. The organization receiving the donation is to complete the bottom of the form validating receipt of the donation, along with the intended use of the donation, and return to the address above for review by the Giving Committee.

Restrictions
The foundation does impose restrictions on certain situations where gifts will not be matched. Those restrictions included gifts to athletic grants or "booster" clubs, athletic scholarships, athletic building programs, tuition or other attendance costs, gifts required for admission, gifts for which the donor receives something of more than nominal value, and class or alumni association dues. It should also be noted that the maximum amount an employee can contribute in any one calendar year is $5,000.00, and the minimum amount eligible for matching is $50.00.
